2012 GVR HIKING CLUB MEMBERSHIP APPLICATION
HIKING CLUB DISCLAIMER: Release and assumption of risk: I understand that during outings of the Green Valley Recreation Hiking Club in which I participate under arrangements of the Club and its agents, certain risks and dangers may arise, including but not limited to the hazards of traveling in the wilderness, the forces of nature, accident and illness. In consideration of the right to participate in these outings and the services provided for me by the GVR Hiking Club and its agents, I have and do hereby assume the risk associated with the outings and will hold GVR Hiking Club and its agents harmless from and defend them against any and all liability actions, suits, claims and demands which could arise out of the outings. This agreement shall serve as a release and assumption of risk for myself and my heirs, administrators, executors, and for all members of my family including any minors accompanying me. Transportation to and from each hike is the individual responsibility of the participating GVR members and their guests(s). The hikes are graded as to difficulty. If my health or physical condition prevents me from performing at the expected level, the Hike Master may limit my participation.

STATE TRUST LAND PERMIT: Due to new rules of the Arizona State Land Department, hiking on State Trust Lands requires an annual $15.00 permit ($20.00 for a family). Many club hikes are on State Trust Land and will require hikers to have a permit. Hikes on State Trust Land will be identified in the monthly hike bulletin with an (SLPermit) tag. Hikers without a permit are subject to a $450 fine.
A permit application may be printed from the club’s web site (www.gvrhc.org) which may then be mailed with a check.
DO NOT attend hikes on State Trust Land without a permit.
